What to expect on your first visit
Once you make the decision, you and your physician to get home health services, A-PLUS staff will come for an assessment at your home and work with you and your doctor to come up with a plan of care. This will include all the services and treatments you may need.
The plan of care will be reviewed on a regular basis by your physician and A PLUS staff. Should anything change in your health condition, together with your doctor, we will make any necessary adjustments to your plan of care.
